I think you are talking about the first VOC HS clearcoats that didn't have enough solvent to dissolve the basecoat. It was common practice (recommended by paint companies) to lay down a very dry, dusty mist coat of metallic basecoat to orientate the metallic particles and avoid tiger striping and mottle.
When the clear is applied, it just can't get the adhesion/bite it needs from the dusty basecoat and the clearcoat delaminates.
It's extremely common to see on factory acrylic cars that have been repaired with two pack as well.
This would be either really shit clear or an extended period between painting the basecoat and clearing it.
