    These types of things are often very tricky to diagnose cleanly. I could list 10 different things that can cause similar pain. Depending on the exact position of the pain, and the exact things that set it off (loaded extension you mention hurts, as does cycling) it's probably a referred pain coming from one of the carpal joints and projecting distally a bit. Could be a TFCC tear (google that shit) or a scapho-lunate dissociation issue (and that). Best person to see would be a hand specialist Physio (they do exist, I work with one) or, if none around perhaps a hand and wrist surgeon who will probably refer you for an MRI straight off before making moves on it.
